Summary

Eligible applicants include clinical and non-clinical health care entities, local governments, tribal organizations, and community-based entities either located in or primarily serving rural Floridians as defined by the RFA. The initiative totals $60,500,000 in Year 1 statewide funding, aims for sustainable, modernized models of rural health care, and requires service launches and core milestones within 90–180 days post-award.

Eligibility

Eligible entities include for-profit or non-profit healthcare providers, community-based and non-clinical organizations, and governmental or tribal entities either located in Florida’s rural regions or actively serving rural Floridians as defined by AHCA for RHTP purposes. MyFloridaMarketPlace vendor registration is required. Medicaid participation is not mandatory.

Requirements Checklist

• Applicants must detail how they will implement all five core initiatives of Bundle One: Mobile Health Units, Community Paramedicine, Remote Patient Telemonitoring, Pharmacy-Based Retail Clinics, and HIE connectivity. Comprehensive proposals need: technical and project plans, provider credentials, evidence of technical capacity, demonstration of collaborative partnerships, detailed budgets using provided templates, and proof of MyFloridaMarketPlace registration.
