Personal Health Accelerator (PHA) Wellness Data & Referral Program

Summary

This Announcement of Funding Availability (AFA) seeks a grantee to serve as a single statewide program operator, accountable for the design, implementation, and operation of an integrated wellness referral ecosystem as a core component of the PHA pillar within the RHTP. The grantee will coordinate provider and community-based organization onboarding, data integration, milestone performance management, and real-time reporting to support measured improvements in health outcomes, cost, utilization, and workforce participation. Funding is for a one-year effort targeting scalable deployment, sustainability, and statewide closed-loop referral integration supporting the Governor's Four Pillars of a Healthy West Virginia.

Requirements Checklist

• Applicants must submit a single integrated application covering full program operations and technology scope. The State will contract with a single accountable entity responsible for program delivery and milestone outcomes. Required components include: closed-loop referral technology (direct or via subcontract), data integration with State platforms, rapid platform launch within 60 days of contract execution, county-level operational and reporting capacity, evidence-based referral completion, engagement and tracking, and sustainability planning. A detailed line-item budget and narrative are required. All CMS and RHTP requirements apply.
