Eligibility

Qualified entity types: hospitals, long-term care nursing facilities, home health care agencies, behavioral health providers, ID/A adult homes, and ID/A intermediate care facilities. Entity must currently operate in Pennsylvania with at least one physical location, be enrolled as a Pennsylvania Medical Assistance (Medicaid) provider, and hold applicable licensure or certificate of compliance from the Pennsylvania Department of Health. Entity must not already be a participant in one of PA's certified HIOs (for onboarding-only tier) and/or must not already have a CEHRT system (for implementation tiers), as applicable to the payment tier requested.

Requirements Checklist

• Funds must be used for updating/implementing a Certified EHR Technology (CEHRT) system and/or becoming a fully onboarded participant in one of Pennsylvania's five certified HIOs (P3N network). All approved funds must be incurred and expended by June 30, 2027; unused funds must be returned to the Department by July 31, 2027. Payments are authorized in the order complete eligibility certifications (with all required documentation) are received — this is a rolling, non-competitive program payment, not a competitive solicitation.
