Pennsylvania lines up FQHC & FQHC Look-Alikes - Modernizing Healthcare Technology and Enabling Interoperability opportunity

Summary

The program is open to Medicaid-enrolled FQHCs and look-alikes operating in Pennsylvania with at least one physical location, in full operation since at least December 29, 2025, and not at risk of closure. One payment per entity is permitted. Documentation requirements and monitoring/audit provisions apply; funds cannot supplant other federal/state awards.

Eligibility

Applicant must be (1) a Medicaid-enrolled provider that is either a Federally Qualified Health Center (FQHC) meeting Section 330 of the Public Health Service Act, or an FQHC Look-Alike that meets federal qualifications under Section 330; (2) currently operating with at least one physical location in Pennsylvania and in operation since at least December 29, 2025; and (3) in good standing and not at risk of closure on date of receipt of funds.

Requirements Checklist

• Funds must be used for purchasing and implementing a CEHRT system in accordance with 45 CFR § 170.315 and for becoming a participant and fully onboarding with one of the Commonwealth’s five certified HIOs (P3N). All entities must complete eligibility certification, submit all required documentation, including vendor quotes, verification letter, budget narrative and worksheet, entity status proof, entity location list, and public funding table (if applicable). All expenditures must be incurred by June 30, 2027. Program payment recipients must fully onboard a P3N HIO within 90 days of implementing CEHRT. Documentation supporting use of funds must be submitted within 30 days of incurring costs; records must be maintained for audit through February 27, 2031.
