Pennsylvania lines up FQHC & FQHC Look-Alikes - Modernizing Healthcare Technology and Enabling Interoperability opportunity

Summary

The Commonwealth of Pennsylvania Department of Human Services (DHS) offers up to six program payments of up to $300,000 each (total cap $1.8 million) for FQHCs and FQHC Look-Alikes to implement certified EHR systems and fully onboard to one of Pennsylvania's five certified Health Information Organizations (HIOs). This is a non-competitive program payment and will be made in the order that complete and eligible applications are received. Eligible use of funds includes purchasing and implementing a CEHRT system and becoming a participant in one of Pennsylvania's five certified HIOs, with all activities and expenditures to be completed by June 30, 2027.

Eligibility

Applicant must be (1) a Medicaid-enrolled provider that is either a Federally Qualified Health Center (FQHC) meeting Section 330 of the Public Health Service Act, or an FQHC Look-Alike, (2) currently operating with at least one physical location in Pennsylvania and in operation since at least December 29, 2025, and (3) in good standing and not at risk of closure on date of receipt of funds.

Requirements Checklist

• Funds must be used for purchasing and implementing a CEHRT system in accordance with 45 CFR § 170.315 and for becoming a participant and fully onboarding with one of the Commonwealth’s five certified HIOs (P3N). Entities must complete eligibility certification, submit all required documentation, incur and expend approved funds by June 30, 2027, and provide supporting vendor quotes, verification letter, completed budget narrative and worksheet, and, if applicable, a public funding table.
