Grant opportunity through Utah Rural Health Transformation Program (RHTP) PATH Initiative 1.5 for rural and frontier Utah health facilities to implement evidence-based behavioral health and primary care integration models. Emphasis areas include collaborative care, workforce and training support, EMR enhancement, and sustainable billing workflows. Program is 100% CMS funded with strict eligibility verification and structured, multi-phase review process as outlined in the RFGA.
Eligible applicants are rural and frontier Utah health facilities. All applicants must meet minimum mandatory eligibility requirements as outlined in Section 1 and 2 of the RFGA, including state purchasing vendor registration.
