The Oklahoma State Department of Health (OSDH) Rural Regional Reorientation (RRR) Program, part of the RHTP, will distribute over $223,476,947 in federal funds (100% funded by CMS/HHS) through competitive subawards up to $4,000,000 per project. The RRR program aims to strengthen sustainability, access, and quality of rural healthcare across Oklahoma (excluding Oklahoma and Tulsa counties), focusing on clinical transformation, partnership development, capital improvements, and innovative care models serving communities under 55,000 residents. Eligible applicants include healthcare and social service providers, Tribal organizations, local governments, hospital systems, FQHCs, Critical Access Hospitals, and partnerships of two or more eligible entities. Funds are reimbursement-only; no cost share or match is required. Awards cannot cover direct personnel compensation or vehicle purchases. Subrecipients must comply with federal (2 CFR 200, Buy America) and OSDH program requirements, including reporting and monitoring. Applications must be received via email by 5:00 PM CT on June 12, 2026.
Eligible applicants are health care provider organizations, federally recognized Tribes and Tribal organizations, local government agencies, Federally Qualified Health Centers, Critical Access Hospitals, social service providers, hospital systems, and partnerships of two or more such entities. Applicants must deliver care to rural populations in Oklahoma counties (excluding Oklahoma and Tulsa counties) and communities under 55,000 residents (see NOFO Attachment A for specific eligibility). Partnerships are strongly encouraged. Applicants must be capable of improving healthcare access, system quality/sustainability, and serving rural Oklahoman populations.
