Eligibility

Eligible applicants include non-profit organizations, accredited educational institutions, and healthcare organizations with demonstrated experience in rural health workforce development. Applicants must have a Certified Ohio Community Health Worker (CHW) on staff by the application due date (June 15, 2026). Organizations must be registered as suppliers in OhioBuys and maintain good standing. Rural-based organizations receive a scoring preference. Out-of-state organizations must have documented partnerships with eligible Ohio entities.

Requirements Checklist
