Eligibility
States are eligible to apply. Program initiatives may benefit and distribute funds to rural health providers, Critical Access Hospitals (CAHs), Rural Health Clinics, Tribal Health Organizations, Federally Qualified Health Centers (FQHCs), EMS organizations, educational institutions, local governments, and community-based organizations.

Requirements Checklist
States must submit a single application covering all five years. Applications must invest in at least three permissible funding uses as described in OBBBA.
A sustainability plan is required.
Application scored on strategy, workplan, outcomes, projected impact, and technical/rural factors.
States must obligate awarded funds by end of federal fiscal year and liquidate by end of the following fiscal year.
Must show progress on initiatives to qualify for workload funding.
Renovations/alterations can’t exceed 20% of funding; provider payments max 15%; EMR replacement max 5%; technical fund max 10%/$20M.
