North Dakota Health and Human Services (ND HHS), through the Rural Health Transformation Program (RHTP), is offering competitive funding for enhancements to existing Electronic Medical Record (EMR) systems, including modules, interfaces, technology, and AI add-ons. The opportunity aims to strengthen service delivery, improve outcomes, and enhance cybersecurity for rural and tribal health providers. Applications must be submitted via Qualtrics by July 29, 2026 at 5:00 p.m. CT. The operating period starts upon grant execution and ends September 30, 2027; all funds must be expended and all enhancements fully implemented by this date. Competitive priority is given to projects clearly describing improvements to patient outcomes, increases in access, and reductions in wait times for care provided to rural and tribal North Dakota residents. Only enhancements to existing EMRs are eligible for funding (EMR replacements receive separate future funding). Applicants must submit a complete application, including responses to all required questions and a properly completed Itemized Subrecipient Budget Template (Excel) with clear cost justifications.
Applicants must serve rural or tribal North Dakota residents. Eligible rural organizations include: critical access hospitals; rural federally qualified health centers; rural hospital systems; non-federally operated Indian Health Service facilities; tribally run 638 health facilities; and long-term care facilities. Entities within or the cities of Grand Forks, Fargo, West Fargo, and Bismarck are considered urban and are not eligible. Projects must benefit rural or tribal residents. Successful applicants will not be eligible for future RHTP EMR replacement funding. Each participating entity must apply separately. Projects must be sustainable beyond the grant period.
