the Rural Health Transformation Program to support Rural Community Health Integration (RCHI) — Initiative 1 of NY's five-year RHTP. RCHI establishes coordinated rural health partnerships to improve care coordination, patient access, financial sustainability, and health outcomes across rural communities. Applications may request up to $500,000 per county for planning and up to $3,000,000 for up to three implementation projects. The minimum geographic unit is a county; multi-county collaborative applications are encouraged. Funded by CMS/HHS as part of a $212,058,207.80 total federal award.
Lead Applicant must be: (1) a registered not-for-profit 501(c)(3) organization or municipal hospital; (2) have a 3+ year history of regulatory compliance with DOH rules; (3) be located in an eligible rural county (see Attachment 1); and (4) include at least one hospital as lead or partner. Each application must have a Lead Applicant, Partner Organization(s), and at least one general or rural emergency hospital. Eligible entity types include: • Hospitals: safety net, general, sole community, rural emergency, and critical access hospitals • Federally Qualified Health Centers (FQHCs) • Certified Community Behavioral Health Clinics (CCBHCs) • Rural Health Networks • Primary care or multi-specialty practices • Diagnostic and treatment centers • Urgent care centers • OMH-licensed facilities • OASAS-licensed facilities • Developmental disability service providers • Long-term care providers • Hospice and palliative care providers • Ambulatory surgery centers • Independent Practice Associations (IPAs) • Emergency Medical Service (EMS) providers Eligible counties: Allegany, Broome, Cattaraugus, Cayuga, Chautauqua, Chemung, Chenango, Clinton, Columbia, Cortland, Delaware, Dutchess, Essex, Franklin, Fulton, Genesee, Greene, Hamilton, Herkimer, Jefferson, Lewis, Livingston, Madison, Montgomery, Oneida, Ontario, Orange, Orleans, Oswego, Otsego, Putnam, Rensselaer, Schenectady, Schoharie, Schuyler, Seneca, St. Lawrence, Steuben, Sullivan, Tioga, Tompkins, Ulster, Warren, Washington, Wayne, Wyoming, Yates
