Eligibility

Eligible applicants are State Medicaid Agencies applying on behalf of rural populations in New Jersey. Applications must benefit defined rural counties or federally/state designated rural census tracts and demonstrate formal collaboration with the State Office of Rural Health and relevant stakeholders. Sub-awardees must primarily provide services to rural residents.

Requirements Checklist

• Applications must include a comprehensive five-year strategic plan for rural health transformation. This plan should address: (1) Workforce recruitment, training, and retention; (2) Targeted support for essential providers including hospitals, behavioral health (CCBHCs), FQHCs, and EMS; (3) Expansion of telehealth, mobile health, and digital care models; (4) Investment in critical hospital and EMS infrastructure; (5) Community-based prevention initiatives and chronic disease management; (6) Defined outcomes and sustainability strategies; (7) Evidence of ongoing stakeholder engagement and partnerships.

Submission Instructions

Applicants must submit an electronic application package via the CMS RHTP online portal. All required forms, attachments, and signatures must be included, following guidelines in the official FY2026 program documentation. Applications are due by the posted deadline.
