The Rural Veteran Health Transformation Request for Applications provides $2,384,094 in federal funds to the Nevada Department of Veterans Services (NDVS) to improve health outcomes for Veterans living in rural Nevada. Supported projects must strengthen rural health systems and address gaps in access to primary care, behavioral health, and coordination with the VA by advancing population health, modernizing health infrastructure, expanding workforce, and implementing health IT solutions. This RFA is issued under the CMS Rural Health Transformation Program, as authorized by H.R. 1 (2025).
Eligibility
Eligibility is limited to the Nevada Department of Veterans Services (NDVS) as the primary applicant for this RFA. Other entities are not eligible for this specific RFA cycle. Proposals must focus on rural Nevada Veterans as defined by HRSA/FORHP rural criteria.

Adhere to CMS/HRSA definitions of rural areas
Projects must be completed and funds fully spent by August 30, 2027

Applications must demonstrate measurable improvement in rural correctional healthcare
Projects must comply with HRSA/FORHP federal rural definition and be located in or serve qualifying rural areas
Letters of Commitment/MOUs demonstrating rural collaborations (required for NDOC as a non-rural entity)
Adhere to federal and state grant compliance and program reporting requirements
Applicants must demonstrate project alignment with Nevada’s Rural Health Transformation Plan.
Complete RHT Tribal Health Application Form (Attachment A).
Provide Budget Plan and Budget Narrative (Attachment B).
Applicants must be registered vendors with the State of Nevada.
Include letters of commitment or MOUs as applicable (for non-rural entities).
Submit application electronically by due date.
Describe rural health care challenge, gap, or infrastructure need
Explain current efforts underway by applicant or partners
Define goals and objectives using SMART format
Detail data collection and reporting plan (number served, services, measured outcomes)
Provide signed MOUs or letters of commitment if applicable
Submit a budget summary with personnel, fringe, travel, supplies, equipment, and other costs
