Eligibility

Eligible applicants include the 28 federally recognized Tribes, Bands, and Councils within the State of Nevada.

Requirements Checklist

• Applicants must: (1) Demonstrate alignment with Nevada’s Rural Health Transformation Plan core goals. (2) Submit a comprehensive project narrative using the RHT Tribal Health Application Form (Attachment A). (3) Provide a complete Budget Plan and Budget Narrative (Attachment B). (4) Be registered vendors with the State of Nevada. (5) Comply with all federal, state, and local procurement and reporting requirements (including site visits, designated fiscal agent, standard accounting system, and periodic financial/progress reports). (6) Complete all projects and expend funds by August 30, 2027. (7) Include Letters of Commitment from partners (if applicable). (8) Submit all required documents via pre-populated email to RHTP@nvha.nv.gov by the application deadline.
