the Rural Health Transformation Program (RHTP). Year 1 grant funding ($3,750,000 total) will support operationally ready EMS organizations delivering post-discharge in-home care for Congestive Heart Failure, Chronic Obstructive Pulmonary Disease, or Joint Replacement Care. Funding is intended for personnel, training, vehicles, technology, clinical equipment, and implementation expenses. Project goals are to improve care quality, reduce costs, and strengthen the rural health workforce. Initial grant performance period is August 1, 2026 – September 30, 2027. Grants may be extended for up to four additional one-year periods, pending CMS and state approval.
Eligible applicants are Vermont state licensed EMS agencies with a formal clinical partnership with a sponsoring hospital or health care facility, or hospitals deploying MIH services through internally employed EMS personnel. Applicants must be operationally ready or nearly operationally ready to provide MIH services as defined in the NOFO. Preference is given to applicants serving rural Vermont (all counties except Chittenden County, unless service to rural areas is demonstrated).
