Minnesota Rural Health Transformation Program (RHTP) provides up to $193,090,618.14 in federal funds (100% CMS/HHS funded) as formula-based, non-competitive grants to eligible rural Tribal Nations for transformation initiatives addressing rural health care access, quality improvement, workforce development, technology-enabled care, chronic disease, and provider financial sustainability. Applications are accepted via ORHPC Online Grants Portal on a rolling basis within posted deadlines. No cost share or match required. Approximately 2% of Minnesota’s total RHTP award is allocated to Tribal Nations. Five-year period of performance from award date. Required reporting and compliance with CMS/HHS grant conditions mandatory.
Eligible applicants are federally recognized Tribal Nations in rural Minnesota. The State of Minnesota is home to 11 federally recognized Tribes; only rural Tribal Nations are eligible under this opportunity.
