The program, a key component of the CMS Rural Health Transformation Program, focuses on expanding access, modernizing technology, strengthening the rural health workforce, and innovating in service delivery, with robust outcome reporting requirements. Priority is given to evidence-based, locally relevant projects for women of reproductive age, pregnant and postpartum individuals, infants, and children through age 18, including those with special healthcare needs in HRSA-designated rural areas. This opportunity is aligned with TDH's 2026–2030 Priorities and Maternal/Infant Health Strategic Plans. No cost share or match is required.
Eligible applicants include rural hospitals and health systems, Federally Qualified Health Centers (FQHCs), non-profit and community-based organizations, local health departments, and other healthcare entities serving HRSA-designated rural communities. Applicants must be registered with the State of Tennessee, as well as with the Secretary of State and, if applicable, registered as a tax-exempt organization with the IRS.
