Eligibility
Eligible facilities include rural health clinics, federally qualified health centers, critical access hospitals, community mental health centers, sole community hospitals, opioid treatment programs, Medicare-dependent small rural hospitals, health centers receiving section 330 PHSA grants, low-volume hospitals, rural emergency hospitals, certified community behavioral health clinics located in rural census tracts, and Medicare subsection (d) hospitals in rural areas.

Prepare up to 60-page project narrative covering proposed goals, target population, transformation plan, initiatives and use of funds, implementation plan/timeline, stakeholder engagement, evaluation and sustainability
