The LIFT Initiative aims to develop, support, enhance, and improve sustainable physical and behavioral healthcare access and outcomes in Utah's rural communities through scalable, data-informed telehealth strategies, reinforced by training and technical assistance. This RFA invites eligible rural providers and partners to apply for funding to implement telehealth and digital care projects — expanded telehealth access and utilization, increased telehealth-enabled sites and specialties, and reduced travel/wait times for rural patients. This RFA is only for telehealth projects; UETN will issue separate RFPs for the Alliance/consortium support portion of the LIFT initiative.
Must be a legal entity (nonprofit, for-profit, governmental agency, tribal entity, educational institution, or healthcare organization) operating in or directly serving one or more of Utah's 25 rural counties. Must demonstrate organizational capacity to manage federal sub-grant funds in compliance with 2 CFR Part 200, must not be excluded from federal program participation (SAM.gov check required), and must agree to all reporting/compliance/programmatic requirements including CMS, DHHS, and UETN program directives (RHTP NOFO, Notice of Award, RHTP Program Directives).
