Supports programs expanding chronic disease management in rural SC, focusing on diabetes, hypertension, sickle cell, and similar conditions. Provides funding to scale specialty care, implement tele-specialty and education initiatives, and train providers to improve coordinated care and outcomes in underserved rural areas. Funds expansion of chronic disease management initiatives to improve health outcomes for rural populations. Supports innovative models and partnerships for chronic care.
Eligibility
Healthcare providers, community-based organizations, local government entities, public health agencies, and nonprofits based in SC serving rural communities as defined by HRSA. Activities must benefit rural SC residents and demonstrate measurable rural impact.

Requirements Checklist
Submit cover sheet, 1-page project summary, 10-page project narrative, detailed budget/budget narrative, disclosure of relevant settlement agreements/claims, and up to 5 pages of letters of support;
Project narrative to include organization background, needs assessment (with data), scope/timeline, monitoring/evaluation, and sustainability;
Must select only one project category per application (Chronic Disease Program Expansion);
Quarterly reporting required;
All funds to be expended by September 30, 2027.
Applicants must comply with 2 CFR Part 200 and 2 CFR Part 300.
