Kentucky is soliciting Kentucky-based training organizations to develop new specialized Community Health Worker (CHW) certificates in four priority areas: Healthy Lifestyle Management, Perinatal and Infant Health, Mental and Behavioral Health, and Dental/Oral Health. Maximum award is $200,000 per certificate, up to $800,000 total for all four, paid via milestone-based payments. Training will be delivered free through Kentucky's TRAIN platform. Applications due August 5, 2026 at 5:00 PM ET; anticipated funding period begins October 7, 2026. Part of Kentucky's Rural Health Transformation Plan, funded by CMS.
