The Iowa Department of Health and Human Services plans to issue RFP PHTHOET27001 in July 2026 for up to seven EMS-led Mobile Integrated Health (MIH) projects in rural Iowa. The goal is to expand access to preventive, chronic disease, prenatal/postpartum, and acute in-home care services and to reduce avoidable emergency department usage. Projects will support EMS agencies with training, telehealth tools, and clinical resources to deliver coordinated, community-based healthcare and to provide consistent in-home care, follow-up, and navigation support.
Eligibility is anticipated for EMS agencies and organizations able to deliver community-based healthcare services in rural Iowa. Definitive eligibility requirements will be specified upon formal RFP release.
