Notice of intent to release RFP PHTHOET27001 for up to seven EMS-led Mobile Integrated Health projects to expand preventive, chronic disease, prenatal/postpartum, and acute in-home care in rural Iowa. Projects aim to reduce avoidable ED utilization and strengthen local healthcare access.

Requirements Checklist
