Notice of intent to release RFP COMPADM26005 for pilot co-located service delivery sites and CHW integration.

Eligibility

Eligibility details will be specified in the forthcoming RFP. Based on the Notice of Intent, applicants will be required to demonstrate collaborative partnerships by submitting legally executed agreements with service delivery partners.

Requirements Checklist

• Applicants will need to propose a service delivery model that includes: (1) Pilot co-located service delivery sites allowing access to multiple appointments for chronic disease prevention and management; and (2) Integration of Community Health Workers (CHWs) for enhanced care coordination. Submission of partner agreements demonstrating collaborative commitment is anticipated.
