The Iowa Department of Health and Human Services will release RFP COMPADM26007 in July or August 2026 to establish Cardiovascular Health Hub and Spoke Networks statewide. The procurement will support rural provider strategic partnerships, telehealth, referral networks, provider recruitment, equipment acquisition, minor renovations, and maternal health care enhancement. Applicants will be required to demonstrate collaborative partnerships through executed hub-spoke agreements. The initiative is part of the $209 million CMS/HHS Healthy Hometowns statewide rural health transformation effort.
Eligible applicants must be able to form partnerships with rural providers and submit legally executed collaborative agreements between hub and spoke sites. Additional eligibility requirements will be detailed in the formal RFP upon release.
