Iowa Department of Health and Human Services intends to release RFP COMPADM26007 in July or August 2026 to establish Cardiovascular Health Hub and Spoke networks statewide. The procurement focuses on rural provider strategic partnerships, telehealth, referral networks, provider recruitment, equipment acquisition, minor renovations, and expanding maternal health care capabilities. Applicants must demonstrate collaborative commitment through executed hub-spoke agreements. This is part of a $209 million CMS/HHS-backed Healthy Hometowns initiative for chronic disease prevention and rural health transformation.
Eligibility will require applicants to be able to form partnerships with rural providers and submit legally executed collaborative agreements between hub and spoke sites. Further eligibility details are pending the formal RFP release.
