The program focuses on reducing chronic disease burden (including diabetes, obesity, hypertension, cardiovascular and respiratory diseases, substance use disorder, and long-term care needs), improving measurable health outcomes, increasing workforce participation, and advancing value-based care. Funded projects are required to demonstrate evidence-based, scalable, and sustainable solutions that address upstream root causes through prevention, nutrition, physical activity, behavior change, and integration with healthcare technology and coordinated care models. The grant is performance-based and tied to achieving State-defined milestones, outcome targets, and timely, real-time reporting. Grantees must coordinate with the State's Connected Care Grid Data Spine, meet all CMS/HHS requirements, and support population-level impact consistent with the Seven RHTP flagship initiatives. All applications must include option-specific narratives, detailed budgets, and county-level service forms for each Program Option applied for. Eligible applicants are: Federally Qualified Health Centers (FQHCs), Community Health Centers (CHCs), hospitals and health systems, integrated delivery systems, non-profit organizations, payers (including Medicaid-aligned entities), local partners (health alliances, home health agencies, pharmacies, multi-partner collaboratives with a single prime applicant), and other third-party vendors supporting clinical/wellness interventions and navigation in WV. Applicants must be non-governmental (state agencies are NOT eligible). Preference given to organizations demonstrating statewide scalability and a verifiable, cash-based private funding match (minimum 1:1 preferred; required to be cash match, verified and committed prior to disbursement; in-kind match is not permitted).
