Requirements Checklist

• Applicants must submit, for each Program Option: (1) a standalone Narrative describing intervention design, engagement/adherence strategies, scalability/sustainability plans, and data integration; (2) a Detailed Line Item Budget (per template); (3) a Budget Narrative-Justification; (4) a Grantee Contact Information Form; and (5) a County-Level Service Form. Q&A Form may be submitted up to 3 business days from AFA posting. Administrative costs are capped at 10% of RHTP funds; in-kind match (including non-cash value, donated staff time, or facility use) is not allowed; all match must be cash, committed, and verified prior to disbursement. Each submission must separately identify a single prime applicant, especially for coalitions/collaboratives, and specify clear roles and accountability. Program eligibility, reporting, cost principles, and privacy/security compliance are outlined in the AFA. All funded interventions must track and deliver outcomes as described in the AFA, with performance-based payments tied to meeting required milestones and indicators.
