Hospital-Based Wellness Equipment funding opportunity (Solicitation 210-122)

Summary

Competitive funding opportunity under the Rural Health Transformation Program (RHTP) Initiative 1: Make North Dakota Healthy Again, administered by North Dakota Health and Human Services. Provides support for hospital-based wellness equipment to improve the health and well-being of staff at North Dakota rural hospitals (Critical Access Hospitals, eligible tribal facilities). Up to 10 awards expected, approximately $50,000 each. Equipment requested should be specific and include brand, model, features, cost information, and justification of need. Application via Qualtrics, one per organization. Performance period starts upon execution and ends September 30, 2027. Funded by CMS/US HHS.

Eligibility

Eligible applicants are rural hospitals in North Dakota, defined as Critical Access Hospitals. Non-federally operated Indian Health Service facilities and tribally run 638 health facilities are eligible. Federally operated Indian Health Service hospitals, clinics, health centers, behavioral health/psychiatric hospitals, and urban hospitals (entities in Grand Forks, Fargo, West Fargo, Bismarck) are excluded. Only one application per organization is permitted for this opportunity.

Requirements Checklist

• Applicants must: 1) Provide background information including organization name, location, population served, project lead/contact (name, title, phone, email), project title and reason for applying, 2) Complete a project narrative with identified need, local data, specific equipment requested (manufacturer/brand, features, cost, model, delivery, installation), justification, and strategies for sustainability, 3) Develop an action plan with milestones (equipment identification, ordering, arrival, installation), timelines, metrics for impact and success, and key personnel, 4) Upload a detailed Itemized Subrecipient Budget Template (Excel) with clear justification. Applications must be submitted via Qualtrics. Applications must support RHTP metrics and evaluation plan. Reporting requirements (progress, impact) apply.
