Eligible applicants are Iowa rural health care providers, rural hospitals, networks, and organizations serving rural census tracts as defined by the Goldsmith Modification. Approximately 47.5% of Iowans reside in rural census tracts; eligibility for individual RFPs may include specific focus areas such as chronic disease, maternal health, and integrated service networks as further defined in solicitation documents. Eligible applicants include Iowa-based provider organizations, healthcare networks, and agencies able to demonstrate legally executed agreements between hub and spoke sites. Rural provider partnerships are encouraged.

Applications and responses are to be submitted via the Iowa HHS grant portal or IowaGrants (https://iowagrants.gov/) as specified in each funding announcement. Select procurements (e.g., RFI# COMPADM26006) must be submitted via email to the Issuing Officer as directed in the RFI document.
