The program aims to improve access and quality of health services for rural Tennesseans by facilitating service integration, care collaboration, and sustainability through digital models and value-based care, with emphasis on perinatal, behavioral health, emergency, pediatric, and chronic care coordination services.
Eligible applicants include rural hospitals, Critical Access Hospitals (CAHs), Federally Qualified Health Centers, non-profit and community-based organizations, and other health care entities serving rural Tennessee communities, as defined by the HRSA Rural Health Grants Eligibility Analyzer. Applicants must be registered with the State of Tennessee and meet all RFA criteria.
