Summary

The Tennessee Department of Health (TDH) is soliciting applications to implement innovative health technology solutions that strengthen healthcare delivery, expand access to care, improve operational efficiency, and enhance patient outcomes across Tennessee’s rural communities. Funded projects may procure, implement, or expand technologies such as telehealth, remote patient monitoring, interoperability solutions, and data infrastructure. Priority is given to initiatives that minimize indirect costs, address critical rural health system needs, and demonstrate sustainability beyond the funding period.

Eligibility

Eligible applicants include rural hospitals, health systems, rural health clinics, Federally Qualified Health Centers (FQHCs), behavioral health providers, and non-profit or community-based organizations serving rural Tennessee. Applicants must be tax-exempt as verified with the IRS, and—except for exempt organizations under T.C.A. § 48-101-502—must be registered with the Tennessee Secretary of State. Rural area eligibility is determined by HRSA criteria (see HRSA Rural Health Grants Eligibility Analyzer).

Requirements Checklist

• Applications must include: (1) Application Form (Attachment 1, up to 5 pages), (2) Competitive Requirements form, (3) Tearless Logic Model (Exhibit 1), (4) Bio sketch(es) or resumes for key personnel, (5) Job descriptions for positions to be hired (if applicable), (6) Letters of Commitment and/or Support, (7) Grant Budget (Attachment 3, with Line-Item Details), (8) State of Tennessee Supplier Maintenance SDDA Access Form, (9) IRS Form W-9. All budgeted costs must comply with OMB Uniform Guidance (2 CFR 200, Subpart E) and State policy. Indirect/administrative costs may not exceed 10% of the total award.
