The Tennessee Department of Health (TDH) will award competitive grants to rural health providers and community partners for the implementation, procurement, or expansion of health technology solutions addressing rural healthcare needs in Tennessee. Priorities are telehealth, remote patient monitoring, interoperability, and technology infrastructure, focusing on data-driven delivery, care coordination, and operational efficiency. Awardees must adhere to all 2 CFR 200 and applicable state procurement and cost principles. No construction, capital expenditures, or asset-value increasing activities are allowed. The total available under RFA #34320-20327 is $33,000,000, distributed across multiple grants for a one-year project period (2026-09-01 to 2027-08-31). Indirect costs are capped at 10%. Quarterly and annual reporting, as well as eligibility verification and audit compliance, are required.
Eligible entities include rural hospitals, health systems, rural health clinics, FQHCs, behavioral health providers, non-profit or community-based organizations serving rural TN, state agencies, and Tennessee local governmental entities. Applicants must hold IRS tax-exempt status, be registered with the Tennessee Secretary of State unless exempt under T.C.A. § 48-101-502, and must serve rural areas as verified by the HRSA Rural Health Grants Eligibility Analyzer. State agencies and government entities are eligible through interagency agreements. All applicants must comply with Tenn. Code Ann. § 4-58-101 et seq. and 8 U.S.C. §1642 for subrecipient eligibility verification.
