Health to Prosperity Chronic Conditions Return-to-Work Initiative

Summary

Funding opportunity through the Rural Health Transformation Program (RHTP) Health to Prosperity (HTP) initiative. Eligibility

Must demonstrate operational capacity, field-based staff deployment, coordination across healthcare and employer settings, and State reporting/compliance. Employers are implementation partners but not eligible for direct grant funds.
