The program seeks to fund provider-led initiatives that advance healthcare access, improve chronic disease management, and realize sustainable technology-enabled care models fully integrated with Connecticut’s health information exchange.
Eligible applicants are healthcare providers or organizations delivering patient care, and local public health departments (municipal entities) operating in Connecticut’s rural areas. Technology vendors may only participate as subcontractors or partners. Eligible types include (non-exhaustive): hospitals and health systems, federally qualified health centers (FQHCs), behavioral health providers, independent medical practices, municipal and state health departments, and academic medical centers.
