The program aims to address rural health disparities in maternal health, chronic disease management, healthcare access, technology modernization, and workforce development. One Regional Coalition per region may apply, representing partnerships among local health departments, hospitals, FQHCs, schools, employers, and other stakeholders. Projects must address at least one funding category and required health outcomes, including pre/postnatal care, chronic disease, and access to care/unmet needs. Success is measured by transformative potential, measurable outcomes, collaborative structure, impact, and sustainability. No cost-share is required; grants are 100% federally funded through CMS. Applications are due by July 1, 2026. Awards per region are based on population, health needs, and application evaluation.
Eligibility
Eligible applicants are Regional Coalitions representing one of Indiana's eight designated rural regions. Each coalition must consist of multiple organizations and stakeholders, including local health departments, FQHCs, rural hospitals, critical access hospitals, behavioral health organizations, EMS, educational/workforce entities, state agencies, payers, community-based organizations, K–12 school corporations, and IHCP-enrolled providers. Only one application per region is permitted, covering rural or partially rural counties as defined by HRSA rural designations.

Requirements Checklist
Submit a single, unified regional application addressing at least one funding category and all required priority health outcomes (pre/postnatal care, chronic disease management, access to care/unmet social needs), or provide formal exception requests with justification. Complete and submit all required templates and attachments: Letter of Intent Worksheet (Template A), Needs Assessment (Template B), Logic Model (Template C), Implementation Workplan (Template D), Primary Subrecipient Budget Workbook (Template E.1), Regional Coalition Grant Budget Workbook (Template E.2), and a Project Narrative (max 30 pages, Template F), as specified in the main RFF. Applications must demonstrate regional collaboration, sustainability, measurable impact, stakeholder engagement, decision-making and accountability mechanisms, and proposed initiatives with justification of budget/capital and provider payment costs. Budgets and workplans are required for the first two budget periods; narrative must explain plans for years 1–5. Monthly, quarterly, and annual performance reports are required.
Applications and all required templates must be submitted electronically via the Indiana Department of Health's grants portal as specified in the main Request for Funding.
