Florida RHTP Application Preparation Resources

Summary

This guidance suite provides in-depth application preparation materials for entities seeking to participate in Florida's RHTP. Resources include: example outcome measure frameworks (quality improvement, financial/operational, EMS, population health); planning templates; work plan appendices; checklists; data sources; best-practices for needs assessment; and state-specific support tools. These documents are to be used for aligning RHTP proposals with state and federal expectation areas, outcome evaluation strategies, and compliance with the FY24-28 EMS Supplement.

Eligibility

All organizations eligible to participate in Florida’s Rural Health Transformation Program (RHTP), including State Flex Programs, rural hospitals, Critical Access Hospitals (CAHs), rural EMS providers, public health organizations, health systems, and other entities engaged in rural health transformation may utilize these preparation materials.

Requirements Checklist

• No funds are directly associated with these resources. Prospective RHTP applicants should use the materials to: 1) Select and design outcome measures for quality, finance, population health, and EMS improvement within their proposals; 2) Structure work plans and narratives using state-prescribed templates; 3) Incorporate benchmarking and evidence-based strategies as shown in FY24-28 planning documents; and 4) Prepare required attachments and work plan appendices for submission to the official application portal once open.
