Family Support Services (FSS) Community Services Grant

Summary

The Texas Health and Human Services Commission (HHSC) is accepting applications for the Family Support Services (FSS) Community Services Grant (CSG) program (RFA No. HHS0016936). This competitive grant funds the provision of coordinated, voluntary crisis pregnancy services, foster care outreach, and family support community services at no cost to participants in nine designated counties in the greater Houston area. Successful applicants will demonstrate capacity for targeted outreach, delivery of direct services, use of data reporting systems, and robust community partnerships. Eligible organizations include Texas-based nonprofits, community-based organizations, and healthcare providers.

Eligibility

Eligible applicants are Texas-based nonprofit organizations, community-based organizations, and healthcare providers capable of providing crisis pregnancy services, foster care outreach, and family support services within the nine-county greater Houston region specified in the RFA.

Requirements Checklist

• Applicants must submit the completed Application Form and all required Exhibits by the deadline. Applications must address the elements in the RFA, demonstrate service delivery capacity, compliance with HHSC standards, and eligibility as described in Section 2.1–2.5 of the RFA. Financial statements (audited or unaudited, see Addendum 3 Q&A) and all required attestations must be included.
