Expanding Worksite Clinics for On-the-Job Support

Summary

Solicits proposals from vendors to support the establishment, expansion, and operation of employer-aligned healthcare clinics across West Virginia, delivering preventive, occupational health, and limited acute care services along with care coordination and referral support to help employees manage their health and reduce work disruptions.

Eligibility

Eligible applicants are: (1) State institutions of higher education; (2) health systems and provider organizations (e.g., integrated systems, occupational health orgs.); (3) for-profit or nonprofit local/regional organizations able to scale statewide; or (4) consortia/partnerships (with a single prime applicant). Demonstrated experience operating, managing, and scaling multi-employer worksite clinic or occupational health models is required. Employers may participate as implementation partners but are not eligible for direct grant funding. Applicants must provide at least two grantee contacts and be registered in SAM/UEI.

Requirements Checklist

• Applicants must submit a RHTP Grantee Contact Information Form (min. two contacts), Detailed Line-Item Budget Worksheet (DLIB), Budget Narrative, County Level Service Form (if applicable), and payer duplication table. Funding may only be used for transformation, expansion, or start-up clinic activities not otherwise reimbursable. Cost-sharing with other federal/state programs is prohibited without RHTP prior written approval. Compliance with CMS and State reporting, data integration, and administrative cost limitations (≤10%) required.
