Implements RHTP by facilitating and supporting clinical investments, distributing and overseeing awards, and providing technical assistance to rural health providers, including hospitals, EMS units, and home health providers statewide.

New Hampshire Community Development Finance Authority

Administers the Rural Health Capital Improvement Investment Program to fund rural infrastructure and minor renovation projects improving access to essential services in rural health and childcare providers.

Governor's Office of New Opportunities & Rural Transformational Health (GO-NORTH)

CMS federal-to-state award for implementation of New Hampshire's Rural Health Transformation Plan and sub-recipient contracts.

New Hampshire Medicaid

New Hampshire Medicaid received CMS federal funding under the Rural Health Transformation Program to support rural hospital and Medicaid transformation initiatives statewide through 2030.

Governor's Office of New Opportunities & Rural Transformational Health (GO-NORTH)

GO-NORTH will act as the lead agency overseeing New Hampshire's RHT program, contracting, and payments to subrecipients, contractors, and staff to carry out statewide rural health transformation initiatives and projects as approved by CMS.

New Hampshire Community Behavioral Health Association

Funds to strengthen and modernize New Hampshire's behavioral health system through coordinated statewide support, including implementation of a common electronic health record platform, workforce development, transition to CCBHC status, and centralization of non-clinical services across the CMHC system.

University System of New Hampshire

Funds to establish statewide rural health workforce career pathways, including the Governor's Health Scholars Award Program (GHAP) and the Common Campus simulation network, to address rural New Hampshire health workforce shortages and expand access to clinical education and training in critical healthcare fields.

Facilitates investments in rural healthcare providers to expand access to primary, behavioral, and specialty care, support workforce development, enable adoption of technology, and implement innovative care and financial sustainability models.

CDFA will manage and distribute RHTP funds to develop a Rural Community Health Infrastructure Program supporting minor renovations, equipment purchases, and infrastructure upgrades for rural healthcare, nursing, and community facilities to expand access to essential services and modernize aging facilities in rural New Hampshire.

Community College System of New Hampshire

Funds to establish and expand rural health care career pathways and create Regional Healthcare Career Guidance Hubs that address critical workforce shortages through education, training, apprenticeships, student support, and career advising across rural New Hampshire.

Implements New Hampshire’s RHTP workforce initiative by establishing regional Healthcare Career Guidance Hubs, paraprofessional training academy, and SIM Lab training to expand rural healthcare career pathways, address provider shortages, double credential attainment, and enhance student supports.

New Hampshire Community Behavioral Health Association

Implements the RHTP community mental health system initiative: (i) establishes shared EHR infrastructure for CMHCs and CCBHCs, (ii) modernizes statewide behavioral health workforce pathways, (iii) strengthens crisis response and integrated care, and (iv) expands access to behavioral health, substance use, and primary care services across rural NH.
