Evaluation of Vermont’s Rural Healthcare Transformation (RHT) Program

Summary

The Vermont Agency of Human Services is soliciting proposals for the design, implementation, and statewide deployment of an evaluation of Vermont’s Rural Healthcare Transformation (RHT) Program. The evaluation will include developing statewide performance metrics, impact analysis, quarterly updates, annual reporting, an interactive data dashboard, and supporting program compliance and mid-course improvement efforts. Total Year 1 funding is up to $1,000,000, supported by CMS. The contract includes clear deliverables and milestone deadlines.

Eligibility

Open to companies with demonstrated experience in statewide health program evaluation, data analysis, and dashboard development. See RFP Section 3: Minimum Bidder Qualifications.

Requirements Checklist

• Contractors must meet requirements as outlined in the RFP, including experience with health data, ability to manage data privacy and state systems, capacity for quarterly and annual deliverables, and ability to develop interactive dashboards on approved State platforms (e.g., Power BI).
