The program will award one organization to manage MCAN implementation, develop an IT platform, create standardized protocols, subcontract with rural clinics, and coordinate care navigation and training, with a maximum total funding of $16.5 million over a 60-month period.
Eligible applicants include, but are not limited to, health systems, universities, specialty providers, health clinics, rural hospitals, Federally Qualified Health Centers (FQHCs), non-profit organizations, community-based organizations, and other eligible healthcare partners. All applicants must be registered with the State of Tennessee.
