Establish the Tennessee Memory Care Assessment Network (MCAN)

Summary

The program will award one organization to manage MCAN implementation, develop an IT platform, create standardized protocols, subcontract with rural clinics, and coordinate care navigation and training, with a maximum total funding of $16.5 million over a 60-month period.

Eligibility

Eligible applicants include, but are not limited to, health systems, universities, specialty providers, health clinics, rural hospitals, Federally Qualified Health Centers (FQHCs), non-profit organizations, community-based organizations, and other eligible healthcare partners. All applicants must be registered with the State of Tennessee.

Requirements Checklist

• Applicants must demonstrate the organizational capacity to establish and manage a statewide MCAN, including the ability to subcontract with rural/underserved clinics, develop and implement a standardized dementia diagnosis protocol, create/implement an IT platform, and coordinate network-wide activities. Detailed receipts, quarterly reports, and compliance with Tennessee Department of Health and federal (CMS/CFR) requirements are required. All organizations must submit an Application, Competitive Requirements, Letters of Commitment from Participating Partners, 2-page Budget Form, completed Supplier Maintenance SDDA Access Form, and IRS W-9 Form.
