Eligibility

Eligible applicants are organizations or entities equipped to advance high-performing primary care practices in Vermont, including but not limited to rural health providers, community health organizations, health networks, and groups with demonstrated experience supporting care coordination, patient-centered practices, and rural health workforce and technology integration.

Requirements Checklist

• Applicants must demonstrate relevant experience in supporting coordinated, patient-centered, and population-focused primary care. Proposal requirements include submission by the deadline, response to RFP evaluation criteria, and agreement to contract terms by the anticipated contract start date.
