the Rural Health Transformation Plan, funded by the federal Rural Health Transformation Program. The Year 1 initiative aims to support eligible Kansas rural providers in scaling proven health technologies with documented impact on care delivery, focusing on sustainability, statewide benefit, and strict compliance. Up to $9.5 million is available in FY26 for 5-10 projects. Applications are submitted via the official online portal by 12:00 PM CT on July 10, 2026. All awards and agreements are contingent on KDHE’s federal funding allocation.
Kansas hospitals, physician practices (including solo practitioners), rural health clinics, federally qualified health centers, local health departments, Native American Sovereign Tribe healthcare facilities, certified community behavioral health clinics, licensed substance use disorder facilities, and licensed long-term care facilities located in rural areas are eligible. Non-rural facilities may participate if projects directly benefit rural communities. Technology companies may act only as vendors, not applicants.
