Summary

The program is fully funded by CMS/HHS to improve rural emergency response and healthcare access.

Eligibility

Eligible applicants are rural EMS and Community Paramedicine entities operating within Oklahoma's RHTP-eligible rural communities and counties. See Attachment A for a full list of eligible communities and counties. Applicants must be legally recognized organizations capable of entering into a grant agreement and fulfilling programmatic and fiscal requirements.

Requirements Checklist

• Applicants must submit completed application materials via the designated SmartSheet portal by the deadline. Required attachments include completed budget and expenditure forms (see Attachment C), subrecipient questionnaire (see Attachment D), evidence of insurance, and the OSDH subrecipient terms acknowledgement. If requesting exceptions to standard terms, applicants must submit the relevant form (see Requested Exceptions template). Applicants must comply with all federal, state, and programmatic requirements described in the NOFO and attachments.
