this program is to support established networks in the eight Delta Region states (Alabama, Arkansas, Illinois, Kentucky, Louisiana, Mississippi, Missouri, and Tennessee) to address unmet local health care needs and improve health outcomes through collaborative activities. Funded networks are expected to implement evidence-based or innovative approaches to improve access, coordination, and the quality of essential health care services in rural communities. Applicants must demonstrate organizational capacity to serve rural Delta areas and propose projects that align with HRSA priorities, including addressing health care disparities, strengthening the rural health workforce, and enhancing health system capacity.
Eligible applicants include domestic public or private, non-profit or for-profit entities, including faith-based and community-based organizations, tribes, and tribal organizations, located in or serving one or more rural communities in the eight Delta Region states. Applicant organizations must represent a rural network or consortium composed of at least three health care provider organizations, each committed to a formal collaborative structure. Applicants may be located in urban areas, but the network must serve rural Delta populations. Prior recipients funded under 42 U.S.C. 254c(f) for the same or similar project are not eligible unless proposing expansion into new communities or services.
