The State of Delaware seeks proposals to establish and operate mobile and fixed rural health hubs that expand access to comprehensive, evidence-based health services—including preventive, chronic disease, behavioral health, and specialty care—for underserved communities in Sussex and Kent counties. Projects must emphasize outreach, workforce development, data-driven equity, sustainability, and report measurable health outcomes.
Eligibility
Eligible applicants include not-for-profit and for-profit organizations, healthcare systems, community-based organizations, consortia, and other qualified entities capable of delivering rural health services in Delaware. Public/private partnerships, and evidence of collaboration with local stakeholders, are strongly encouraged.
Applicants must provide a detailed workplan, project narrative, budget and narrative, project timeline, and required State of Delaware forms (including vendor information, non-collusion statement, confidentiality and exceptions forms, business references, and, if applicable, subcontractor form). Proposals must demonstrate service delivery capacity in rural settings, address both mobile (Component A) and/or fixed-site (Component B) interventions, and align with goals for access, equity, workforce, and sustainability.
