The program funds professional services to implement sustainable, replicable rural health interventions integrating nutrition-based strategies for chronic disease management. Activities include recruitment of community health workers and registered dietitians for Kent and Sussex counties, telehealth and EMR integration, culinary medicine teacher training, and robust data/evaluation strategies. Federal funding is available for non-food expenses. The aim is outcome-driven improvement in rural food security, chronic disease, and establishing long-term payment/reimbursement infrastructure.
Eligible applicants include nonprofit organizations, hospitals, health systems, universities, and other entities capable of delivering rural health services and nutrition interventions in Kent and Sussex counties. Preference for organizations with track record in workforce development, telehealth, EMR integration, and community partnerships. Registered Dietitians must hold Delaware state licensure.
