Crisis to Care: Establish or Expand Community Paramedicine Program

Requirements Checklist

• Applicants must submit a clearly labeled application packet (maximum 25 pages, excluding attachments), using the official Attachment A form. Required components include agency and applicant information, current EMS agency licensure (KBEMS), program overview and evaluation plan, scope of services, detailed implementation timeline (with milestones and deliverables), methodology narrative including partnerships (with emphasis on behavioral health), barriers and sustainability plan, attestation of compliance, EMS provider roster, authorized agency representative signature, detailed FY27 budget using Attachment B, and letters of support (e.g., medical director). Submission must be via email to jimmie.hampton@ky.gov with subject line 'RHT CP RFA'. Late or incomplete applications will not be considered.
• Applicants must submit a completed application, project narrative, Attachment A (budget template), required organizational documentation, and evidence of partnerships. Applications must describe service delivery capacity, performance metrics, and financial and operational sustainability for the funding period.
• Submit a completed application packet including the narrative, budget (using template Attachment A), and letters of support (strongly encouraged).
• Narrative must address executive summary, project description, populations/scale, partnerships, service gaps, care coordination, special populations, workforce/training, access/nondiscrimination, sustainability plan, budget and timeline.
• Organizations must report on RHTP implementation progress periodically throughout the funding period and participate in program monitoring/data sharing.

Submission Instructions

Email completed application (Attachment A, max 25 pages + required attachments) to jimmie.hampton@ky.gov with subject 'RHT CP RFA'. Submit completed applications and required documents by email to john.landis@ky.gov by the stated deadline. Email completed application packet to Breanna.mcginnis@ky.gov by July 3, 2026.
