This competitive RFA supports Kentucky's Rooted in Health: Rural Dental Access initiative, part of the CMS Rural Health Transformation Program (RHTP). It provides funding for mobile dental service providers to expand access to restorative and definitive dental care in rural, high-need Kentucky communities. Funding is federally provided (CMS/HHS) and prioritized for nonprofits operating mobile dental units in partnership with public health and community organizations, with awards contingent on performance metrics. This Request for Applications (RFA) invites Medicaid-enrolled, Eastern Kentucky serving 501(c)(3) nonprofit organizations focused on behavioral health services to apply for funding under the Rural Health Transformation Program (RHTP). The funding supports participation as spoke organizations in a statewide telebehavioral health hub, aiming to expand access to psychiatric consultation, crisis-adjacent behavioral health support, and care coordination for rural communities. Grants prioritize high-readiness applicants implementing programs to strengthen crisis response, continuity of care, and integration with primary care providers. Eligible applicants are non-profit and publicly funded EMS agencies operating in the Commonwealth of Kentucky. Agencies must hold a current, active Class Ia or Class Ib EMS agency license from Kentucky Board of EMS (KBEMS) and remain in good standing throughout the award period. Agencies must provide 911 advanced life support (ALS) responses within Kentucky. Eligible applicants are nonprofit organizations capable of operating mobile dental units in rural Kentucky, with demonstrated partnerships with local public health and community organizations. Applicants must be able to expand access to restorative and definitive care for high-need rural populations. Medicaid enrolled, Eastern Kentucky serving 501(c)(3) nonprofit organizations with a demonstrated focus on behavioral health services. FQHCs and RHCs will have an opportunity to participate as PCPs in the model. Applicants must have or must establish a vendor code with the Kentucky Secretary of State and the Finance Cabinet if awarded.
