Creating Healthier Rural Communities Through the End Zone for Chronic Disease Prevention

Summary

the Rural Health Transformation Program (RHTP), this opportunity is open to rural hospitals, health systems, FQHCs, and community-based organizations serving maternal/infant health or coordinating rural health events. Applications may include activities in up to three eligible counties and must demonstrate how projects align with evidence-based interventions for nutrition security and chronic disease risk reduction.

Eligibility

Eligible applicants are rural hospitals, health systems, Federally Qualified Health Centers, non-profit and community-based organizations, public health agencies, and other qualified organizations with demonstrated experience in maternal/infant health or health-related event coordination serving rural Tennessee. Applicants must be registered with the State of Tennessee and may request activities covering up to three eligible rural counties per application. See Amendment 1 for clarifications.
