Requirements Checklist

• Applicants must design, implement, and scale hub-and-spoke models under the Connected Care Grid. Requirements include demonstrating integrated alignment with the broader RHTP ecosystem, scalability and sustainability of the care model, mandatory statewide data integration, and measurable impact on chronic disease outcomes, ED utilization, successful referrals, and workforce participation. Applicants must submit a line-item budget and narrative and comply with CMS and State reporting and milestone requirements. Maintenance of Effort applies: funds may not be used to supplant existing activities or staffing.
