Kansas's CHW+AFIM grant program, funded by the federal Rural Health Transformation Program, provides over $1 million in competitive awards to rural healthcare organizations for integration of Community Health Workers (CHWs) and expansion of Accountable Food is Medicine interventions. The initiative supports validated screening for health-related social needs and chronic disease risk, sustainable CHW employment, and increased coverage for evidence-based food-as-medicine programs. The Request for Applications targets rural Kansas healthcare organizations to develop tailored billing workflows, deliver AFIM interventions targeting chronic conditions (primarily diabetes and hypertension), collaborate with KDHE, and submit required documentation. Approximately seven subrecipient organizations statewide will be funded in this initial project year. Award range: $95,000–$150,000 per organization.
Federally qualified health centers, rural health clinics, hospitals, local health departments, Native American Sovereign Tribe healthcare facilities, certified community behavioral health clinics, FQHC look-alike clinics, and primary care clinics located in rural Kansas areas. Must possess UEI, FEIN, and SMART Vendor ID.
