Behavioral Health Promotion Community Grants

Summary

This program is part of North Dakota's five-year Rural Health Transformation Program, designed to strengthen rural health workforce, promote preventive care, bring high-quality care closer to home, and connect technology, data, and providers.

Eligibility

Eligible applicants are organizations capable of advancing behavioral health promotion in rural and tribal North Dakota communities, including nonprofits, local government, tribal entities, community-based organizations, and educational institutions. Entities must complete North Dakota Secretary of State registration and state vendor registry requirements. SAM.gov registration and a UEI are mandatory for awards under federal funds.

Requirements Checklist

• Proposals must be creative and evidence-based, focus on behavioral health promotion, and address mental health, suicide prevention, and/or substance use/misuse prevention for people not yet identified as needing treatment. Grantees must complete North Dakota Secretary of State registration and/or be registered in the state vendor registry and provide a W9, as required. For federally funded awards, registration at SAM.gov and an active UEI are required before contract execution or payment.
