3–6 awards of up to $2M each to integrate behavioral health into primary care settings in rural Utah. Requires collaboration from 3+ local agencies. Estimated total funding up to $15,000,000 over 5 years. Eligibility

Eligible applicants must be rural healthcare providers or organizations serving rural communities in Utah. Collaboration required among three or more agencies, including healthcare, behavioral health, and local government entities.

Requirements Checklist
