The Alabama Rural Health Practice Initiative is a competitive funding opportunity under the Alabama Rural Health Transformation Program (ARHTP), funded by the Centers for Medicare and Medicaid Services (CMS) Rural Health Transformation Program and administered by ADECA. A total of $6,935,000 is available for competitive subawards to support projects that expand rural clinic capacity, recruit and retain rural healthcare workforce, implement telehealth, and develop school-based health clinics. This initiative aims to advance rural healthcare access, innovation, and the long-term sustainability of health services across Alabama's rural communities. Eligible entities are: (1) Rural Health Clinics located in or proposing to serve rural census tracts in Alabama; (2) Federally Qualified Health Centers (FQHCs) currently operating rural satellite sites; (3) Critical Access Hospitals; (4) Public or nonprofit entities with demonstrated ability to provide primary care or behavioral health services in rural settings. Applicants must show a history of serving rural and medically underserved populations and maintain compliance with state and federal regulations. Projects must benefit rural, medically underserved areas as defined in the NOFO.
